NHS Greater Manchester’s Population Health Team have drafted a ‘GM Primary Care Inclusion Health Toolkit’ to help commissioners and providers of primary care to think about inclusion health.

The toolkit could support practices with their Salford Standard AI1 projects, PCN health inequalities projects or other work to tackle health inequalities within the practice / PCN.

The Population Health Team are seeking volunteers to use the toolkit, and to provide any feedback to inform future iterations.
