Practices are requested to complete the attached participation agreement to confirm whether it wishes to deliver the Learning Disabilities Health Check DES in 2024/25.  

Completed participation agreement forms should be returned to gmicb.gpassurancework@nhs.net by 30th June 2024.

Please check before submitting both pages of the participation agreement that it has been highlighted ‘Yes’ or ‘No’ to delivery on Page 1 and Page 2 contains signature(s), P Code and practice name.
