Practices are requested to complete the attached participation agreement to confirm whether it wishes to deliver the Minor Surgery DES in 2024/25.

Please can you check before submitting both pages of the participation agreement that you have highlighted either Yes/No to delivery on Page 1 and on Page 2 have included signatures, P Code and practice name. As part of the 2024/25 sign up process practices are asked to complete the attached supporting evidence and declaration form to provide NHS Greater Manchester with the assurance that GPs delivering the DES are eligible aas per the service specification and RCGP guide to GP Clinical Extended Roles. Completed participation agreements and supporting evidence/declaration forms should be returned to gmicb.gpassurancework@nhs.net by 30th June 2024 

A GP that is currently not accredited to provide the DES Minor Surgery by NHS Greater Manchester should not be delivering the service under the DES until accreditation has been obtained.

To obtain accreditation the GP is required to complete the NHS Greater Manchester Minor Surgery accreditation application form (attached) and return to enhancedserviceses1@nhs.net Completed applications will be reviewed for approval by Chief Medical Officer, NHS Greater Manchester Integrated Care.
